If you require any vaccinations relating to foreign travel you will need fill out a travel questionnaire which you can either pick up at reception or downloading below and handing in to reception. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to complete your form at least 6 weeks before you travel to give the nurse time to review your records and advise on any vaccinations and associated costs and go through any travel advice about the area you are visiting. Vaccines have to be ordered as they are not a stock vaccine and any vaccinations which incur a cost will need to be paid for before they are ordered. Your appointment to have the vaccines needs to be at least 2 weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
